How to Use This Order Form Template

01

Add your details

Company name, address, and where completed orders should go. Switch on the header fields your customers need: PO number, ship-to, delivery date, payment terms.

02

List your products

One row per SKU with description, unit of measure, case pack and price. Leave quantity blank: that is the customer's job.

03

Download and send

Excel keeps the totals formulas, CSV drops into any system, and print gives you a PDF to email or a paper copy for the sales rep's bag.

Send the Excel version to customers who order from a desk, and the PDF to the ones who print, write and scan. Either way, keep one current copy and re-send it when prices change, so nobody orders off last year's sheet.

What a Good Order Form Includes

Every field on an order form exists because someone once shipped the wrong thing without it. These are the ones that save the most phone calls.

  • Your SKU on every line

Customers who order by your item number get the right product. Customers who order by description get whatever the picker guesses.

  • Unit of measure and case pack

State whether the price is per case or per each, and how many units are in a case. This one column prevents most quantity errors.

  • Current price

Print the price the customer will be invoiced. If prices change quarterly, date the form so old copies are easy to spot.

  • PO number field

Larger accounts will not pay an invoice that does not carry their PO number. Ask for it on the form, not by email after the fact.

  • Bill-to and ship-to

Separate blocks, because the warehouse that receives the goods is often not the office that pays for them.

  • Requested delivery date

Gives your team a target and stops the call asking where an order is when it was never promised for that day.

  • Payment terms

Net 30, COD, prepay. Printing them on the order form avoids the dispute at invoice time.

  • Where to send it

One email address or fax number for completed orders, printed on the form, so orders do not land in a rep's personal inbox.

Frequently Asked Questions

Is this order form template free?

Yes. Build the form, download it as Excel or CSV, or print it to PDF. There is no signup and nothing is uploaded: the file is generated in your browser.

How do I make a wholesale order form in Excel?

List your SKUs with description, unit of measure, case pack and wholesale price, add your company details, and download the .xlsx. The workbook already has the quantity column, a line total formula on every row, and an order total at the bottom, so your customer only types quantities.

What is the difference between an order form and a purchase order?

An order form is the seller's document: your products, your prices, sent to customers so they can order. A purchase order is the buyer's document, issued from their system with their PO number. Many wholesale customers fill in your order form and attach their PO number to it, which is why the template has a PO number field.

Should the form show cases or units?

Both, clearly. Put the unit of measure (CS, EA, BX) and the case pack on every line so the customer knows that 2 in the quantity column means 2 cases of 12, not 2 bottles. Mixed-up units are the most common reason an order ships wrong.

Will it open in Google Sheets or Numbers?

Yes. The .xlsx is a standard Office Open XML workbook and opens in Excel, Google Sheets, Numbers and LibreOffice with the formulas intact. The CSV opens anywhere but carries values only, without formulas or formatting.

What do I do with the forms customers send back?

Most come back as a PDF, a phone photo, or the spreadsheet with quantities typed in. Someone then keys them into the ERP. You can drop a completed form into the free PO Extractor to get the lines back as structured data, and OrderSync does the same thing automatically for every order that lands in your inbox.
